Your front desk closed at six. The enquiries did not.
Picture the enquiry that arrives at seven in the evening. Someone found your clinic, decided they want an appointment, and sent a message through your site or your WhatsApp. Nobody is at the front desk. By the time someone opens it in the morning, she has booked with the clinic down the road that answered at seven fifteen.
You will never know she existed. Nobody rings back, because nobody knew there was anything to ring back about, and there is no record of the patient you did not get. That is the whole problem with how clinics lose enquiries: the loss is invisible, so it never gets fixed.
How often does an enquiry go unanswered?
More often than most owners believe. Zenoti surveyed 1,107 US adults in September 2025 about how they book and communicate with clinics. Three findings matter if you run a front desk:
- 79 percent have abandoned a booking rather than finish it
- 73 percent say they are more likely to choose a clinic that is reachable 24 hours a day
- 82 percent say they are more likely to rebook where there is after-hours support
Read the first one as a person. Someone wanted to give you money, hit friction, and left quietly. They did not complain, they just booked somewhere that made it easy.
Why do the first five minutes decide it?
Because the enquirer is talking to several clinics at once, and the one that replies first sets the frame. This has been measured properly. Harvard Business Review audited 2,241 US companies by sending each a web enquiry and timing the reply:
- 37 percent responded within an hour
- 16 percent responded between one and 24 hours
- 24 percent took more than a day
- 23 percent never responded at all
And the effect of that hour is large. Firms that tried to reach the enquiry within an hour were nearly seven times as likely to qualify the lead as those who waited even one hour more, and more than sixty times as likely as those who waited a day or longer.
Nearly a quarter of the market does not reply for over a day, and almost another quarter never replies. That is the competition you are actually up against at seven in the evening, and most of them are not even trying.
What do we actually set up?
An agent that answers on the channel the enquiry already arrived through, and does the three things a good front desk does at its best:
Answers first
A reply in seconds, not hours
Whatever time it arrives. It answers in your words, says what you actually offer, and never invents a price or a clinical opinion. If it does not know, it says so and takes a message rather than guessing.
Qualifies in the thread
The questions your front desk asks anyway
What treatment they are asking about, whether they are a new or returning patient, when they are free, and anything you need before the appointment. The same questions, asked the same way, every time, at eleven at night.
Books the slot
Straight into your calendar
It reads your real availability and books the slot, then confirms. No round trip, no "we will call you back tomorrow". The appointment exists before the conversation ends.
Hands a person in
Anything that needs a person
Complaints, clinical questions, unusual requests and anyone who asks for a human get passed to you with the conversation so far. The agent's job is to handle the routine and escalate the rest, not to pretend it is a clinician.
Which channels does it work on?
WhatsApp is live today, and your website chat, Slack, iMessage and the phone line you already advertise run on the same agent. You keep the number patients dial and the website you already have; the agent sits on top of them rather than replacing them.
Some channels need a little setup on your side, and we tell you exactly what that is before you commit to anything: iMessage needs either an Apple device or Apple's business approval, and the phone connects through the carrier you already use. We will confirm what is possible for your clinic, in writing, before any work starts.
If the phone is where you lose people, there is a cheaper first step than a voice agent: a text back to every missed call, asking what they needed and offering a time. It is worth doing whether or not you go any further with us.
When is this a waste of your money?
The catch is that none of this helps if your front desk already answers every enquiry within a few minutes and your phone is properly covered in the evenings and at weekends. In that case you do not need any of it, and we would rather say so than sell you a system your team already runs by hand. The clinics that do need it are the ones where enquiries arrive outside the hours somebody is free to reply.
Will an AI receptionist put patients off?
The evidence says no, and this surprised us too. In the same survey of 1,107 US adults, 71 percent said they were comfortable with an AI receptionist, including 80 percent of people aged 45 to 60. The age group owners tend to worry about is not the obstacle here: four in five of them said they were comfortable with it.
What people actually mind is being ignored, and being told something that turns out to be wrong. An agent that answers immediately and admits when it does not know something beats a voicemail box every time.
What does this actually change in the numbers?
We looked for a reputable industry average for response time and could not find one worth printing: most of the figures circulating come from companies selling the fix, with no survey behind them. So we use your own baseline instead, which is a better number than anyone else's average.
It changes the denominator. Clinics usually measure how many enquiries they got and how many became appointments, which quietly assumes every enquiry reached a person. It did not. Once every enquiry is answered and every conversation is logged, you can see the real figure: how many arrived, how many were answered, how many were qualified, how many booked.
That is the number worth fixing, and until the agent is answering you cannot see it.
What does it cost?
There is no price on this page, because there is no single price for it. The cost is quoted against the scope of work: the channels your enquiries actually arrive on, the calendar it has to book into, and how much of the qualification you want it to handle before a person takes over. You get that in writing before anything is built, and we will not charge you for a channel your enquiries do not use.
What we will not promise
We will not promise a number of extra bookings. We will promise that no message on the channels we run goes unanswered, that every conversation is logged, and that you can see how many arrived and what happened to each one. Before the agent goes live we measure your current response: how fast enquiries are answered today, and how many never get answered at all. That baseline is what any later claim has to be judged against.
How do we start?
With your own numbers, not a demo. Before anything is built we look at how enquiries reach you today: which channels they arrive on, how quickly somebody replies, and how many never get a reply at all. That tells us whether the agent is worth building.
Then we agree the three things it has to get right, which are yours to decide: the treatments you actually want more of, the questions your front desk asks every patient, and the calendar it books into. It goes live on one channel first. You watch it answer for a while, correct anything it says wrong, and only then do we add a second channel.
Frequently asked questions
What is speed to lead?
Speed to lead is the time between an enquiry arriving and somebody replying to it. The hour matters because the patient is usually still deciding in it: she is messaging more than one clinic, and the first useful reply is the one that shapes the choice. Reply tomorrow and you are no longer part of the decision.
How quickly should a clinic answer a new enquiry?
Within minutes, not hours. In that same audit 37 percent of companies replied within an hour, 24 percent took more than a day, and 23 percent never replied at all. The enquiry is usually contacting several clinics, so the first reply sets the frame for the conversation.
Do patients mind talking to an AI receptionist?
Most do not. In a September 2025 survey of 1,107 US adults, 71 percent were comfortable with an AI receptionist, including 80 percent of those aged 45 to 60. The thing people object to is being ignored or being given an answer that turns out to be wrong, which is why ours says when it does not know something.
Which channels can the agent answer on?
WhatsApp is live today, and Slack and web chat go live during setup. Our own client agent has been answering in a real WhatsApp group since 6 October 2026, which is why we lead with it rather than with a demo. WhatsApp, your website chat, Slack, iMessage and the phone line you already advertise. iMessage and phone need a little setup on your side, and we set out exactly what that is before you commit to anything.
Will it book appointments or just take messages?
It books. It reads your real availability and puts the appointment in the calendar before the conversation ends, then confirms. Anything that needs a person, such as a complaint or a clinical question, is passed to you with the conversation so far.
What happens to enquiries it cannot answer?
They reach a person. Anything outside what it has been told is passed to your team with the whole conversation attached, so nobody has to start again. Guessing is the one thing it is explicitly stopped from doing, because a confident wrong answer about a treatment or a price costs you the patient and the review.
How do we know it is working?
You get a monthly page showing four numbers for the month: enquiries received, answered, qualified and booked. The same four were recorded before launch, so the first page already has something to sit against rather than starting from nothing.
Does this replace our front desk?
No. It covers the hours and the moments the front desk cannot: after close, during appointments, weekends and busy periods. Your team keeps the conversations that need a person, and stops losing the ones that only needed a reply.
